Description This is the raw dataset provided by the informaticist, after we added missing BMI and surgery dates that were available in the chart, calculated time interval variables, changed age > 89 years to 89 years, and then removed all other PHI (dates, zip code). The variables in this dataset have not been recoded or imputed. The codebook for the variable names is provided on the second sheet of the excel file.
