Abstract
Chronic and recurrent workplace stress can profoundly impact the physical, mental, and emotional health of first responders, with downstream consequences for injury risk, quality and safety of patient care, and organizational performance. Mindfulness in Motion is an evidence-based multi-modal workplace intervention integrating mindfulness practices, gentle movement, and facilitated group discussion to reduce stress and burnout while enhancing resilience and work engagement. This study evaluated the effectiveness of Mindfulness in Motion among first responders using a virtually delivered, synchronous format supplemented by a companion smartphone application designed to support ongoing practice and engagement.
Fifty-one first responders participated in the mindfulness intervention across three cohorts over nine months. Self-reported outcomes, including perceived stress, resilience, work engagement, and musculoskeletal pain, were collected at baseline, immediately post-intervention, and six months following completion of the eight-week program.
First responder participants demonstrated significant reductions in perceived stress and musculoskeletal pain, along with significant increases in resilience and work engagement, immediately following the intervention. At six-month follow-up, improvements were sustained for perceived stress and resilience, with first responders continuing to report significantly lower stress and increased resilience compared to baseline.
The Mindfulness in Motion intervention was associated with improvements in both physical and psychological stress-related outcomes among first responders, with benefits sustained for at least six months post-intervention. Future research should examine longer-term effects across diverse geographic and organizational settings to assess generalizability. Additionally, studies are needed to determine the mechanisms underlying sustained benefits, including the role of accessibility and usability of the companion smartphone application, as well as the independent and synergistic effects of app-based and synchronous intervention components.
This submission reflects the raw dataset that includes de-identified demographic indicators for the cohorts of first responders who participated in an eight-week worksite mindfulness intervention supplemented by the use of the companion smartphone app; use of the app continued for six months after the eight-week mindfulness program was completed. The dataset (REDCap data exported to Excel) and code (STATA Software) used to run the analysis on pre-, post-intervention, and 6-month post-intervention outcome measures of perceived stress (PSS-10), resilience (CD-10), work engagement (UWES-9), and musculoskeletal pain (mNMQ) are also included. The REDCap Excel file is labeled with each outcome measure and question numbers for the measures. The responses (number) for each outcome measure question align to the Likert scale range specific to each measure; this de-identified file can be used freely.

Description of the data and file structure
This prospective cohort study evaluated the effects of the Mindfulness in Motion (MIM) intervention on perceived stress, resilience, work engagement, and musculoskeletal pain among community-based first responders. Three cohorts participated in eight weekly, synchronous virtual MIM sessions delivered during the workday between September 2023 and May 2024. The intervention was supplemented by a companion MIM smartphone application. Following completion of the synchronous sessions, participants retained access to the MIM smartphone app for six months that included evidence-based mindfulness content, guided meditations, body scans, and experiential practices. Outcome measures were collected at baseline (pre-intervention), immediately post-intervention, and at 6-month follow-up.

Human subjects data
All human subjects provided informed consent prior to participating in any study activities. Once data analysis was complete, all participant identifiers (name, email, and contact information) were removed from the data sets via the Safe Harbor method.
First responders, in three cohorts, completed a brief demographic questionnaire at baseline that collected information on age, gender, and years of work experience. Outcome measures for perceived stress (PSS-10), resilience (CD-RISC-10), work engagement (UWES-9), and musculoskeletal pain (mNMQ) were collected one week before participation in the MIM intervention, one week after the last day of the eight-week intervention, and again six months after completion of the MIM intervention. Mindfulness in Motion smartphone app usage, aggregated by cohort and measured over the 8-month study period, was measured as total hours of use during the study period. In addition, engagement with specific categories of MIM content was also assessed.
Participant demographics were analyzed descriptively using mean and range for age and years of experience as a first responder; gender was described as percentages. Engagement with the MIM smartphone application was monitored throughout the eight‑month study period for each of the cohorts. Engagement time was calculated using means, and app analytics were used to identify the most frequently viewed video content as well as the average number of minutes spent viewing that content across all cohorts.
To explore the association between the eight-week MIM intervention and the key outcomes of interest, perceived stress, resilience, work engagement, and musculoskeletal pain, paired samples t-tests were used (α=0.05). Effect sizes were calculated with Cohen’s d. All analyses were conducted using STATA/SE 17.0. The results were compared pre- and post- MIM intervention to detect significant differences in outcomes, with sustainability of outcomes examined 6-months post-intervention.
